Individual Eligibility

You qualify for a discount if you participate in any ONE of these government benefit programs:

  • Supplemental Nutrition Assistance Program (SNAP)
  • Medicaid
  • Federal Public Housing Assistance (FPHA)
  • Veterans Pension and Survivors Benefit
  • Supplemental Security Income (SSI)

OR

  • Your income is at or below 135% of the federal poverty guidelines

Household Eligibility

A household is a group of people that share income and expenses.

  • You are only allowed to get one Lifeline discount per household.
  • If you share housing, complete the Household Worksheet, available at LifelineSupport.org. You can contact us at 435-622-5007.

How to show you are Eligible:

You may need to show proof that you qualify for Lifeline, such as:

  • A copy of your SNAP or program letter
  • A copy of your pay stub or tax return to prove your income is at or below 135% of the federal poverty guidelines.

Holiday Trade-In Event – (Nov. 29 - Dec. 31)

Trade in your eligible smartphone and receive $300 additional credit on top of your trade-in value. Use the total discount towards the purchase of a select new phone that matches or exceeds the value of your discount.

How it works:

  1. Trade in your old phone: The trade-in value will be assessed based on your phone’s model and condition.
  2. Receive $300 additional credit: Your trade-in value will be boosted by up to a $300 credit.
  3. Apply the discount: Use your total discount (trade-in value + $300 credit) to lower the price of a new phone. The discount must be applied to a phone of equal or higher value than your total discount.

Fine Print:

Only one trade-in device per customer. No minimum trade-in value is required to qualify. The maximum credits applied cannot exceed the retail price of the device being purchased. The additional $300 credit will be applied over 24 monthly bill credits. The phone must be purchased on a monthly installment plan and on a current wireless package. If the customer disconnects service, becomes delinquent on payments, or exercises early upgrades or device payoffs before 24 months, credits will end, and the remaining balance is due. Holiday Trade-In Policy

Customers purchasing a phone as a holiday gift have until January 10, 2025, to bring in their trade-in device. At the time of purchase, the new phone will be set up on installments, the $300 promotional credit will be applied, and the customer will be responsible for any down payment required. If the trade-in phone is not brought in by the January 10, 2025, deadline, the $300 promotional credit will be forfeited, and the customer will be responsible for the entire cost of the new phone. Once the trade-in device is received, the assessed trade-in value will be applied as an additional down payment on the phone, and the installment plan will be adjusted accordingly.
